Athletic Paperwork (REQUIRED): The 2024-2025 FinalForms is NOW Open for Enrollment.  

§  Athletes MUST have a completed online athletic packet in FinalForms, which includes an uploaded KHSAA Sports Physical and Insurance Card(front/back) before they can tryout or practice.  All the required information can be found on Woodland’s Athletics website. 

§  If you are NEW to Final Forms, you will need to go to https://kenton-ky.finalforms.com/ and create a new account to register your child.  If you need help, all instructions can be found under the Parent Playbook link on Woodland’s Athletic website. 

§  If you have already created an account, just sign in. DO NOT create a new account.  At the beginning of each year, you will be required to revisit your forms from the previous year.  Make any updates that are needed and then sign showing you have reviewed all the documents.

§  Please email the Athletic Director with any questions/issues.  Final Form accounts must be approved before you can participate in tryouts or practices.

 §  The KHSAA Sports Physical is NOT a 6th grade school physical.  It is a separate form that the doctor must complete, and then it must be uploaded to Final Forms. A copy of this form can be found on Woodland’s Athletic website.
